Usually a rock hits the glass and breaks the glass.
When the object hits the glass air gets in (black spot), the air wants out. Temperature change and vibration make it get out, which causes it to crack.

In most case, yes, but it depends on where the rock hit the glass, how hard, and how long the crack is.
This is a frequent issue. Here’s what happens almost all the time, a rock hits the glass, and the driver sees a chip that has already been repaired. To avoid this assumption, judge the damage from the outside of the car.
Windshield glass is made with two layers of glass with a plastic in-between which is pretty much indestructible. I’m not an engineer though and can’t guarantee it doesn’t.
There are instances where the rock will break both the outside and the inside of the glass, these can not be repaired.
Unfortunately the black part of the windshield is very hard to notice a chip, it’s also the most sensitive part of the glass.
